  • IN 5/2017 I had a bilateral Doppler carotid u/s done. No significant stenosis and no plaque seen in ICA and BCA was reported.
    Owing to fleeting blindness/blackout sensations (no falls or significant disruption to activity) I had the study repeated.
    In 7/2018 the conclusion was "stenosis of the common carotid arteries of 50-69%, stenosis of internal carotid artery on the right of less than 50%,stenosis of the left internal carotid artery of between 50-69%". My GP said he doesn’t know much about carotid artery disease as had only one or two other such patients. He did say I would end in a Nursing Home because nothing much can be done. I said I wanted to use any preventative measure available and he referred me to a neurologist. The 4 local neurologists in private practice have had mixed patient reviews and I would not feel confident going to them. The only neurologist that I know to trust is a hospital based specialist. My GP is reluctant to make referrals there. Question: how serious is the U/S assessment? The deterioration in stenosis over 14 months seems extraordinary. Which is the most relevant evidence based specialist to see? Co-incidentally, the local vascular surgeons either do not list carotid artery disease as a "interest" (would have preferred "experience") or again patient reviews are very mixed as to their outcomes and empathy (actually lack of). I live in Canberra.

    Thank you. The answer regarding your clinical circumstances and proven carotid artery disease is simple.
    The severity of carotid disease in your circumstances is moderate and as such would be managed with the best medical therapy which includes low dose of daily Aspirin (100 mg), statin therapy and blood pressure control. I would recommend MRI of the brain with DWI to make sure moderate right ICA stenosis is not resulting in distal embolistaion causing TIA (mini strokes).
    I’d seek referral to a Vascular Surgeon. You will require further follow up to make sure your carotid disease doesn’t progress further.
