  • Crps (complex regional pain syndrome) in both feet considering having both feet amputated, doctors tried everything else. what do you think?

    This is a question that is often asked by patients frustrated with the pain with CRPS. There has been some developments with more evidence emerging in CRPS management with some thechniques like Graded Motor Imagery, Transcranial Magnetic Stimulation, Spinal Cord Stimulator.Having said that the treatment trialled till date needs to be reviewed in a Tertiary Pain Centre -Multidisciplinary setting to try and optimise it before moving on to second line treatment options.
    There is not a huge degree of evidence to support amputation will 'fix' the problem, essentially the changes that continue to drive the pain in CRPS happen in the Central nervous system-Brain and Spinal Cord so if amputation is planned purely to get rid of pain it does not work. However if there is an insensate foot, with skin damage, trophic changes etc. there is more evidence in favour of amputation. This needs to be discussed with your Pain Specialist and also a Vascular/Plastic/Orthopaedic Surgeon.

    my denpax patches don't stick, what should I do?

    Denpax (generic fentanyl Patches), dont stick well if the skin is oily.Apply after shower on a clean dry skin and hold down with the palm of your hand for 30-40seconds. The warmth of the hand helps the adhesive set better.
    If this does not work, ask your Doctor to prescribe the brand Durogesic in the same strength (original brand, better adhesive technology), if this does not work it would mean considering changing to another medication.
