Midline flexion is the worst movement for any sort of mesh repair whether it be open or laparoscopic. it is the mesh fixation which is the critical step for reducing hernia recurrence, and it is thought that early (less then 6 weeks) mesh migration is the cause for recurrence in the first 2 years after repair.Pilates and core muscle exercises certainly have a role to play in long term reduction of abdominal wall herniae, but not in the initial phase post repair where you do not want the fixation points to be stressed.
I generally am happy for cardiovascular exercise (swimming, jogging, cross trainer) to begin after 4 weeks, but avoid rowing machines or reclining/recumbent stationary cycles. i generally tell my patients to avoid lifting more than 5kg for the first 6 ekes post-op and then gradually re-introduce weights programme after that.

Most Surgeons would use 4 weeks post surgery as a general guide to not putting undue pressure / strain on the operative site. Its really a guide. Most times pain will tell you if you are doing too much- i.e. pain at operative site = cut back on whatever i am doing.
